MEDICAL WAIVER FOR TRYOUTS

Tryouts will be held on BOTH Saturday, Sept. 26 AND Sun. Sept. 27. If you CANNOT attend both sessions, please sign up for the MAKEUP DATE at the bottom (Tues. Sept. 29)

12 and UNDER - Sat. Sept 26th 9-11am AND Sun. Sept 27th 1-2pm

​

13-14u - Sat. Sept 26th 12-2pm AND Sun. Sept. 27th 2:30-3:30pm

​

15-16u - Sat. Sept. 26th 3-5pm AND Sun. Sept 27th 4:30-5:30pm

​

17-18u - Sat. Sept. 26th 6-8pm AND Sun. Sept. 27th 6-7pm

MAKEUP TRYOUTS (ONLY IF YOU CANNOT ATTEND BOTH SESSIONS ABOVE FOR YOUR AGE GROUP):
 

All Ages - Tues. Sept. 29th 6:30 - 9pm

​

ALL UNPAID ENTRIES MUST PAY $50 cash or check at the door. Early Bird discounted ended on Thursday at 5pm.  Feel free to walk up. Make sure you fill out Tryout form. TSHIRT included with tryout, no guarantee on sizes.
